INSTRUCTIONS:
The student must fill out the entire form except for the Grade, Instructor's Signature, Advisor's Signature, and the Office Use fields (the fields are the boxes or blanks on the form). The student must then obtain both the course instructor's signature and their advisor's signature.

Then go to:

  1. Financial Aid Office (TH 108)
  2. Registrar's Office (TH 204)
  3. Business Office (TH 220)

The Add/Drop Slip below has been filled out as an example.
