INSTRUCTIONS:
The student must fill out the entire form except for the Grade, Instructor's Signature, Advisor's Signature, and the Office Use fields (the fields are the boxes or blanks on the form). The student must then obtain both the course instructor's signature, current grade, and their advisor's signature.

Then go to:

1. Financial Aid Office (TH 108)
2. Registrar's Office (TH 204)
3. Business Office (TH 220)
