General Information |
Services are available at no charge to all students. Health Services will assist students in making appointments with dentists or medical specialists. The cost for such additional medical services is the financial responsibility of the student. Over-the-counter medications and supplies are available at the discretion of the doctor. |

Vaccine Information |
Oklahoma State Senate Bill 787 requires all college students to be provided with detailed information on the risks of meningococcal disease. Also, all students must show proof of immunization for Hepatitis B and MMR. Please fill out the certificates below and send your immunization record, or proof of immunization to USAO. |
