fill out and complete using
typewritten or in black ink only, and return it to USAO's Office of Records and
Registration at the address below.
- First time entering freshmen
must submit a high school transcript and ACT or SAT scores.
- College transfers with less than 30 credit hours
must submit a high school transcript and ACT scores.
- College transfers with more than 30 hours
must submit official transcripts from each institution attended and ACT
scores.
