If you are not applying online, print USAO's EZ-APP, fill out and complete using typewritten or in black ink only, and return it to USAO's Office of Records and Registration at the address below, along with your $15.00 (US) application fee. In addition:

First time entering freshmen must submit a high school transcript and ACT or SAT scores.
College transfers with less than 30 credit hours must submit a high school transcript and ACT scores.
College transfers with more than 30 hours must submit official transcripts from each institution attended and ACT scores.
